PATH

Peer Assistance through Transition and Healing

PATH supports adults with intellectual and developmental disabilities as they transition from institutional or highly supported environments into greater community participation and independence. The program uses peer mentorship, pairing participants with mentors who have lived experience to provide culturally responsive and relatable guidance, recognizing the importance of trusted relationships and representation in helping participants navigate real-world challenges, strengthen self-advocacy, and build confidence.

Relaunching Late 2027: PATH's initial implementation served 10 adults with intellectual and developmental disabilities with support from 5 peer mentors. Standing Tall Collective plans to relaunch PATH in late 2027, building on that foundation.
